3) a copy of any internal mandatory information governance training that you give to staff which was written in the last 2 years including presentation slides and videos and any other media
4) a copy of any instructions given to staff members to reduce data security breaches, for example double checking work which was written in the last 5 years
5) a list of any policies implemented in the last 2 years within the organisation to help reduce the environmental impact that the organisation has?

Camden and Islington NHS Foundation Trust are pleased to respond as below.

 

1) Please can you send me a copy of the current subject access request
cover letter that you use –

2) a copy of the last 5 dpias completed

We are unable to provide completed DPIA as it involves external suppliers
and review of the security level policy and other documents belonging to
third party. Therefore exempt as part of Section 23 of FOI Act 200-
Information supplied by, or relating to, bodies dealing with security
matters. We have however, attached a DPIA template. Please note, our DPIA
is currently being updated.

3) a copy of any internal mandatory information governance training that
you give to staff which was written in the last 2 years including

The Data security training is provided by Health Education England. You
can request a copy of the e-learning from HEE. Further information

The Trust this year has updated the following policies which make
reference to environmental impact:

 

•             Cleaning policy .Updated July 2019.

•             Pest control policy . Updated May 2019.

•             Waste management policy .Updated July 2019.

•             These policies are regularly monitored to ensure compliance
and the trust has engaged with its FM contractor ISS to improve the waste
management stream.

•             In addition the trust Hard FM Manager has introduced
controls to manage down the usage of electricity and heating ,
specifically on the St Pancras site, the reduction at this stage is not
quantifiable or in a policy .
