We write further to your email of 17^th December made under the terms of
the Freedom of Information Act 2000.

Please accept our sincere apologies for the delayed response. This is not
indicative of the service we provide and we are sorry if our delay has
caused you any inconvenience.

York Teaching Hospital NHS Foundation Trust's response against each
element of your request is shown below and attached.

 

1)    Please can you send me a copy of the current subject access request
acknowledgment AND response letter that you use  

As attached.

The acknowledgments relates to a couple of categories ie Acknowledgement
of request or acknowledgment of paperwork.  We would if necessary send out
a form for requested ID.   This is also broken down into living and
deceased categories due to two different laws that govern the specific
categories. I have attached the letters for all.

 

Regarding the response letter, it is dependent on what we are responding
to in the request ie viewing request, no trace of records, access refused
or the letter we send with the completed paperwork, we have attached all
and again they come under living and deceased categories.

 

2)    a copy of the last 5 dpias completed

This is being withheld under the S22(1) provision of the FOI Act, as the
information you requested is due for future publication and we hope to
have these available on our public website before the end of the financial
year.

 

3)    a copy of any internal mandatory information governance training
that you give to staff which was written in the last 2 years including
presentation slides and videos and any other media

Copies of our IG worksheets and supporting slides for all new starters are
attached.

 

4)    a copy of any instructions given to staff members to reduce data
security breaches, for example double checking work which was written in
the last 5 years A copy of our staff guides is attached. A copy of our
Staff Guides and relevant screensavers is attached. In addition, there
have been ad hoc reminders in internal Trust newsletters and
communications.

 

5) a list of any policies implemented in the last 2 years within the
organisation to help reduce the environmental impact that the organisation
has?

 

In the last three months, this Trust has launched a carbon and cost
reduction programme that will benefit the Trust for many years to come
following on from a review which highlighted that significant low-cost
carbon savings could be made if the Trust embraced more sustainable
working practices.

The Trust’s sustainability team has been working with waste, recycling and
energy efficiency specialists WRM, who are experts in sustainable
development. The company was chosen as they have previous experience with
other trusts and healthcare organisations.

 

This initiative is designed to reduce the Trust’s environmental impact by
changing staff behaviours and the culture of the organisation through more
efficient ways of working to cut cost and carbon.

 

There are nine sub-projects within the scheme and staff are being engaged
in all of these projects with the first two being a teleconferencing pilot
and the start-up of a  ‘green champions’ network to generate ideas on how
to make the workplace greener and to help and support the changes. The
teleconferencing pilot is aimed at senior managers who drive more than 50
miles a week to get to meetings to demonstrate how the Trust can save time
and money. Managers are being asked to switch to teleconferencing on at
least one of their regular meetings.

 

The seven other sub-projects are:

• Material Reuse – developing a Trust-wide system for reuse of furniture
and other equipment

• Building Energy Management System (BEMS) optimisation – reviewing how to
better monitor and control heating and cooling through the current Trust
system

• Installation of temporary sub-meters for wards and departmental areas to
promote awareness of energy consumption

• A new energy reduction working group who review energy performance,
drive forward energy saving initiatives and improve coordination between
the various departments with responsibility for energy use and management

• Energy kaizens will apply improvement tools on chosen areas to provide a
truly bespoke system that works for the Trust

• A quarterly briefing to unions to gain support and promotion of the
sustainability project across union membership

• Senior sustainability champions who lead the way by providing
information and briefings throughout the whole of the Trust supported by
Jane Money, Head of Sustainability.
