Thank you for submitting a request for information under the Freedom of
Information Act 2000.  Please find the response below in red font. This
response covers the hospitals and services managed by the Royal Free
London NHS Foundation Trust, namely the Royal Free Hospital, Barnet
Hospital and Chase Farm Hospital and associated group services.  

 

The response

1) please can you send me a copy of the current subject access request
acknowledgment AND response letter that you use

Please see attached Word documents.

 

2) a copy of the last 5 dpias completed

Please see attached x3 completed and approved DPIAs. We are unable to
release another completed DPIA due to the sensitive clinical content. The
trust also has a number of other DPIAs which are currently in draft
format.

 

3) a copy of any internal mandatory information governance training that
you give to staff which was written in the last 2 years including
presentation slides and videos and any other media

The trust’s mandatory information governance training is embedded within
training software and is not in a format that can be extracted and shared.

 

4) a copy of any instructions given to staff members to reduce data
security breaches, for example double checking work which was written in
the last 5 years

Attached is a copy of the trust’s Patient and Staff Confidentiality Policy
which is available to all staff and included within Information Governance
training, a new version of this this policy is currently under review and
due for approval January 2020.

 

5) a list of any policies implemented in the last 2 years within the
organisation to help reduce the environmental impact that the organisation
has?
