Patients suffering severe harm

Dear Pennine Care NHS Foundation Trust,

Looking at the Trust's recent Annual Reports, it seems there have been no incidents which have resulted in severe harm to patients.

As many patients are completing suicide, it is difficult to understand why there are no patients suffering severe harm.

Can you confirm that there have been no incidents resulting in severe harm to the patient over the last six years? Otherwise please provide the statistics for each year.

What criteria do management of the Trust use to define severe harm?

For each of the last six years, please advise :-

How many patients have become paralysed to the lower body
How many patients have become paralysed to both upper and lower body
How many patients have suffered brain damage
How many patients have damaged their internal organs
How many patients have lost limbs
How many patients have lost the use of a hand or limb through nerve damage

Is the Trust able to state how many former patients over the last six years are now being permanently cared for in a hospital, nursing home or care home because of injuries they have sustained as a result of self harm or attempted suicide?

For these purposes, patients are those persons who have had contact with the Trust in the 12 months prior to the incident resulting in harm.

Following receipt of your request, searches were conducted within Pennine
Care NHS Foundation Trust (the Trust) to locate the requested information
and I can confirm that the information requested is held by the Trust.

 

1. Can you confirm that there have been no incidents resulting in severe
harm to the patient over the last six years? Otherwise please provide the
statistics for each year.

 

Please note that the information provided in the Trust Annual Reports
relates to patient safety incidents which the Trust is required to report
externally in accordance with nationally agreed processes for the NHS in
England and Wales to the NRLS (National Reporting & Learning System).  A
patient safety incident (PSI) is defined as: ‘Any unintended or unexpected
incident(s) that could have or did lead to harm for one or more person(s)
receiving NHS funded healthcare’.

*These are the incidents we have uploaded to the NRLS and attached a
severe harm grading.  However we have identified a general data quality
issue with the harm grading of PSI submitted to the NRLS by the Trust, as
this is not in accordance with the  NRLS Data Quality Standards.  This is
because we have attributed a severe harm code to PSI incidents submitted
to the NRLS based on the level of severity of the incident rather than
considering if the harm sustained was directly attributable to an act or
omissions by Trust.  We are therefore presently liaising with the NRLS to
clarify the issue identified and agree what further action is available in
regards to the data already submitted.
