Thank you for your email.

 

I have attached a recent response to a substantially similar request the
ICB has replied too.  NHS Northamptonshire ICB has one provider, DHU, for
its geographical area of Northamptonshire.

 

I hope this provides the information sought however please clarify is that
is not the case and especially please clarify what information is sought
in the first bullet point and are the other bullet points asking the same
information as each other?
