Number of reports of various medical conditions

Dear Public Health England,

I would like to know the number of cases of various medical
conditions reported in certain areas across the UK over a period of
time.

The conditions I am interested in are rickets, scurvy, TB, Scarlet
Fever and Whooping Cough.

I would like to know the number of cases for the last ten years -
2004-2014 to date, broken down per number of cases per year.

Please could I have the information broken down per council area or
Clinical Commissioning Group (or another suitable format which the
information is already stored in) for the West Midlands,
Nottingham, Leicester, Stoke, Shrewsbury, Derby, Hereford,
Worcester.

I would also like the figures for the same periods of time and for
the same conditions for the UK as a whole.

An example of the format would be

Scurvy 2
