Neurology Inquiry - Terms of Reference.

Dear Department of Health (Northern Ireland),

1. Following Brett Lockhart QC'S comments to patients on 21/06/22 that he wished he could have changed the terms of reference;
a) Did Brett Lockhart QC at anytime ask for these to be changed/altered/extended?
b) Has the TOR for this inquiry ever changed?
c) Who decided the terms of reference for this inquiry? Please list all individuals involved and provide a copy of all correspondence/minutes/records.
d) Who selected Brett Lockhart for this inquiry? Please provide a copy of all correspondence/minutes/records.

2a) Whose decision was it to exclude patient engagement from the Terms of Reference and why?
b) Please provide a copy of the all party letter that was signed requesting patient participation in this inquiry.

3a)The department claims that the reason this inquiry was established as a non-statutory inquiry initially, was due to the collapse of the Northern Ireland Executive. However, less than 6 months after work began on the inquiry, the powers of the executive moved in to the hands of the DOH. Please clearly outline, why at this stage didn't the Department immediately convert this to a public inquiry?
b) What date did the executive powers move into the hands of the DoH and how many witnesses had given oral evidence to the inquiry at that time?
c) By the time the Health Minister was appointed in January 2020, how many witnesses had given oral evidence to the inquiry?

4a) Brett Lockhart QC comments to patients on 21/06/22, that by the time this was converted to a public inquiry in December 2020, he had then spoken to 206 witnesses and in the interests of "time", it was decided that the hearings shouldn't be public. Who made that decision? Please provide a copy of minutes/correspondence and any other documentation as it pertains to this?
b) Once again, why wasn't patients engaged with to ask them if they would have preferred their rights to hear evidence given to this inquiry and challenge it/submit counter evidence , to be removed in favour of "time"?
