Misappropriation of HM Treasury Funds.

Dear Coventry Teaching Primary Care Trust (PCT),

Can you please answer the questions below in relation to a GP in the Cheylesmore area of Coventry, whom is financed by your Primary Care Trust.

1) Can you tell me and my associates, under exactly WHAT, Laws/ Regulations allows a GP whom has not seen a Patient within their Practice for several years to;

A) Discuss his/her Patient with a CPN, whom is totally unknown to a Patient, whom is also NOT in receipt of any CPN services whatsoever, WITHOUT their patients Knowledge or Consent ?.

B) Pass and share their Patients records onto the Caludon Centre, Mental Health Hospital whom the Patients are totally UNKNOWN TO, AND NONE of the Hospital Doctors or staff have ever had any contact whatsoever with the Patient AND without the Patients Knowledge or Consent ?.

C) Arrange an appointment for his/her Patient, with a CPN, for a Home Visit. Where the GP has NOT seen the Patient and neither has any other medical person, AND the Patient has NOT been in Hospital AND has NOT had any Medical Intervention of any sort. Without the Patients Knowledge or Consent ?.

D) Place a record of a Transfer/Discharge from a Mental Hospital into a patients medical records, where the Patient has NEVER been in a Mental Hospital, without their patients Knowledge or Consent ?

E) Remove and Alter Patients records, AFTER the Patients have obtained copies of Originals, without the patients Knowledge or Consent ?.

F) Deny allegations of wrong doings to Practice Managers and Patients and later make admissions when PROVEN to be LYING ?

Our response to each of your questions, as listed in your information request, is as follows:

1A There is no information held by the PCT which states the circumstances where these situations may apply and what course of action a GP should or should not take. Discussions regarding registered patients with other services are the decision of the GP and are based on clinical need for which formal consent is not generally sought.

1B Patient records are shared where there is a clinical need for other services to become involved in the care of the patient or in the management of a condition.

1C There is no information held by the PCT which states the circumstances where these situations may apply and what course of action a GP should or should not take. Discussions regarding registered patients with other services are the decision of the GP and are based on clinical need for which formal consent is not generally sought.

1D Management of patient records is the responsibility of all services where a patient has been in contact or accessed care. When a service notifies a GP practice of a patient being in receipt of care or other intervention the practice is required to record this within the patient record.

1E Maintenance of patient records is the responsibility of all services where a patient has been in contact or accessed care. When a service notifies a GP practice of a patient being in receipt of care or other intervention the practice is required to record this within the patient record, there may be changes to diagnoses or other aspects of the patient's care which are then required to be updated as part of this maintenance to ensure accuracy of a record.

1F There is no information held by the PCT which refers to these situations, it would be appropriate for a patient to raise these concerns with the Practice Manager to ascertain the facts and to resolve any queries or misunderstandings on behalf of all involved.

The GP has made admissions of passing and sharing Patient information to the <<Caludon Centre>> [Mental Hospital].
NONE of this is recorded in his Patients Records. Meaning, the GP Surgery in the Cheylesmore area of Coventry, are covertly discussing Patients and passing Patient records, behind their Patient's back. Furthermore, the Caludon Centre and the Swanswell Point have records of Mental Health Diagnosis of Patients, where they do NOT share the information with their Patients GP's. So they are all making admissions of refusals of a Duty of Care. The <<Coventry City Council>> place records in their finance office of people having carers, yet NO-ONE has ever visited the Patients and NO-Care has been given. So HOW can they obtain money for a service that has NEVER been provided ?....

Funding goes into the "pot" and is utilised by those who have access to it how they deem appropriate. It is also known that for each referral made the referee receives a payment commensurate with their position. Therefore, the higher they are up the scale the higher the payment received. It makes economic sense for the referee to make as many referrals as he or she can. The records are encrypted so are not able to be viewed by everyone which makes the process easier.
