Dear Cabinet Office,

With reference to the Cabinet Office memo from Ben Lyon to former Cabinet Secretary, Gus O'Donnell dated 14 May 2009, disclosed as a result of a successful freedom of information request on 16 November 2016, I request disclosure of minutes and records for the meeting held on 15 May 2009 to discuss 'next steps' in the setting up of the Iraq Inquiry; attended by Jeremy Heywood, Bill Jeffrey, Peter Ricketts, Minouche Shafik, John Scarlett, Simon McDonald and Margaret Aldred. This meeting was, according to the record, chaired by Gus O'Donnell.

Dear FOI Team Mailbox,
I request an internal review.
I note that the disclosed Cabinet Office document referred to in the original request states that it contains an annotated agenda for the meeting of 15 May, which was due to take place at 17.00pm and was scheduled to last for an hour.
I ask that you answer the following questions in drafting this internal review;
i) will you confirm whether this meeting took place at the time and date stated on the memo- (or possibly it was held at a later date?)
If the meeting took place;
ii) were minutes taken at the time and were records kept?
If minutes were taken;
iii) were these minutes later destroyed or lost in some way?
iv) Were these minutes and records later transferred to another government department or location within government? Which government department or location were they moved to?
If you claim that the meeting did not take place, what was the reason for its canellation after such detailed instructions were given about it in the memo of 14 May 2009, which lies at the heart of this freedom of information request.
