Dear Sir /Madam

I respectfully request all information held relating to the recent case involving the King family and Ashya King.
Including but not limited to;

All information released to police and media.
Details in consultations regarding information communicated about proton beam therapy.
Any Mention of an emergency protection order.
Records of written and email requests from Mr King.
Details of any meetings regarding Ashya Kings care in relation to the family being Jehovah's witnesses.
Any previous complaints against Dr Gary Louis Nicolin GMC Reference Number 3458819.

It would also be useful to see details of the feeding machine / equipment given the claims about how difficult it is to charge. In many cases such equipment is placed in the hands of patients and carers to enable them to take more ownership of their own care. What was special about this case that warranted the alarm?

In addition to the request for evidence of any previous complaints against Dr Gary Louis Nicolin GMC ref. 3458819, please add his colleague:-

- Dr Ramya Ramanujachar, GMC ref. 4597267

whom, I believe, was also directly involved in the case of Ashya King.

I would also like to add to the above, the request to release all information and evidence to us,

including the evidence upon which Southampton Hospital relied in their instructions to the Crown Prosecution Service to issue a Domestic or European Arrest Warrant,

evidence of Southampton Hospital's instructions to Portsmouth City Council to apply for a court order making Ashya King a ward of the High Court,

and evidence of claim relied upon and given to the media in support of their search to "Find Ashya", as the public information campaign slogan was styled.

The annotations can't be seen unless the staff log in to their own accounts, it would be useful to make your own requests as they can only deal with Shirley's request, annotations are only really for added information the public know of to assist the original poster.
I also guarantee that although SGH have already breached the patient confidentiality of Ashya by speaking freely to the press, they will use the data protection act in order not to answer these questions, they will speak only to fit their own agenda, good luck with your findings
