Investigations into the deaths of patients with a learning disability

NHS Walsall Clinical Commissioning Group did not have the information requested.

Dear NHS Walsall Clinical Commissioning Group,

Please would you provide the following information for your CCG:

1) the number of deaths of patients with a learning disability that occurred in the last three years (by years defined as 1/4/16 - 31/3/17, 1/4/17 - 31/3/18, 1/4/18 to present).

2) the number of independent investigations into the deaths of patients with a learning disability that you have commissioned in the last three years (defined as above)

3a) the number of deaths of patients with a learning disability that have resulted in an inquest in the last three years (defined as above)

3b) of those inquests the number that were an Article 2 inquest

The Clinical Commissioning Group (CCG) has no right of access to Personal
Confidential Data (PCD). PCD cannot lawfully flow from any providers or GP
practices directly to the CCG unless there is explicit consent from the
patient, a need for sharing information for the direct clinical care or it
is covered under section 251 regulations.  Please ensure that if you
send/forward an email to the CCG that it does not contain any PCD.

The law required that Walsall CCH responded to my request by 16 April, so the notification of a delay due to local elections purdah (referred to as a general election) on 26 April is bordering on ridiculous.
