Information relating to the forthcoming rollout of the SCR

Dear Somerset PCT

I would like to make some requests under the FOI Act regarding the rollout of the Summary Care Record.

For the purposes of the Act, the date of your receipt of this request will be 12th March 2009.

From the splendid "Jennifer's Journal" page of Somerset LMC's March newsletter, I understand that, as regards the SCR, "propaganda will be going out to patients over the next few weeks and they will be given the opportunity to opt out."

• Please would you confirm if your PCT really is a "fast follower", or "potential fast follower" PCT as regards the SCR rollout.

• Please would you send me the minutes (draft or approved) of any PCT meeting (e.g. Project Board, Project Implementation team) regarding the Summary Care Record/NCRS/Healthspace created or updated between 1st October 2008 and 27th February 2009.

• Please would you send me any PCT SCR/NCRS/Healthspace Highlight Reports (draft or approved) created or updated between 1st October 2008 and 27th February 2009.

• Please would you send me any PCT SCR/NCRS/Healthspace Issue Logs and Risk Logs (draft or approved) created or updated between 1st October 2008 and 27th February 2009.

• Please inform me as to what meetings take place within the PCT on a regular basis regarding any forthcoming SCR Rollout, and what updating documents (such as logs or board/implementation team updates) are produced on a regular basis.

Please could you also send me:

• The latest version of the patient (PIP) letter that you are sending out, or planning to send out to residents about the SCR.

• The latest version of the SCR opt out form that patients will be using in your PCT.

As per Section 4a of Part 1 of the FOI Act
( http://www.opsi.gov.uk/acts/acts2000/ukp... ) I would like the information in question held at the time when my request is received (draft or otherwise), except that account may be taken of any amendment or deletion made between now and the latest time by which the information is to be communicated to me, being an amendment or deletion that would have been made regardless of the receipt of my request.

I would be grateful if you would be kind enough to send me the requested information promptly and in any event not later than the twentieth working day following the date of receipt of my request - that is by then end of the working day of 9th April.

If my request is denied in whole or in part, or specific items within the responses are withheld from disclosure, then you must justify all deletions by reference to specific exemptions of the act, as per Section 17 of the Act
( http://www.opsi.gov.uk/acts/acts2000/ukp... ). Where you rely on a qualified exemption to withhold disclosure, you are obliged to consider the public interest in your decision and the refusal notice must explain not only which exemption applies and why, but also the public interest arguments addressed in reaching the decision.

I wish to receive the information by email; I believe such a request would be reasonable in these circumstances. As per Section 11 of Part 1 of the FOI Act ( http://www.opsi.gov.uk/acts/acts2000/ukp... ) I would be grateful if you would "so far as reasonably practicable give effect to that preference"; if you determine that it is not reasonably practicable to comply with this preference then you are required by law to notify me of the reasons for your determination.

The “return address” (to which the information that I have requested be should be sent ) is the email address that this particular request originates from.

I would be grateful if you would kindly acknowledge receipt of this request as recommended by the ICO (“It would be good practice to acknowledge receipt of requests and to refer to the 20 working day time limit, so that applicants know their request is being dealt with”).

Information Act 2000.  I have now obtained the relevant information and
have responded to your request below.

Question 1) NHS Somerset has been accepted for ‘fast follower status’ for
the Summary Care Records and HealthSpace rollout.

Question 2) Please find attached the draft minutes of the first Project
Board and Project Team meetings. Please be aware that these minutes are in
draft format and are subject to alteration.

Question 3) Please find attached internal reports for the project.

Question 4) Please find attached the Project Log which details issues,
risks, scores and lessons learned. 

Question 5) Project Board meetings will take place every other month. Team
meetings will be held monthly.

The following Project Documents are produced using the Prince2 Project
Methodology and will be updated accordingly.

Question 6)  Please find attached a copy of the latest version of the

Question 7) The opt out form will be available to download from our
website (currently under development) and will be the Connecting for
Health version which I have attached above. 

If you are at all dissatisfied with this response you may appeal to NHS
Somerset and we will ensure that an independent review is undertaken or,
alternatively, your next course of action is to request that the
Information Commissioner’s Office review our decision in writing at:
FOI/EIR Complaints Resolution, Information Commissioner’s Office, Wycliffe
House, Water Lane, Wilmslow, Cheshire, SK9 5AF.

It is still a section 10 DPA request form, which is no longer being used - in fact, the ICO ordered CfH to stop using s10 almost 2 years ago.

Thank you for your email, dated 9 April 2009, regarding the opt out form. 

In our response to your request under the Freedom of Information Act 2000,
dated 8 April 2009, we stated that the opt out form would be available to
download from our website but that the website was currently under
review.  Once the development of the website has been completed I can
confirm that the opt out form available will be the latest Connecting for
Health version from their web site at
