No records have been recoded and that we are not exactly sure what is
meant by re-validation. But if an audit doesn’t meet the required 90%
primary diagnosis accuracy, we would do a further audit on the
speciality/individual 3 months later but there is no amendment to the
coding

Do you have an automated process for updating General Practitioner
information changes?

If not, how are the updates managed and what is the average delay in the
updating process?

Without clarification it is difficult to answer this. Is this in reference
to the information that is held on a particular GP practice as an
organisation – i.e. the GPs who work at a particular surgery or the
specific GP registration attached to a patient record?

 

How many whole time equivalent team members process incorrectly delivered
letters?

For the letters returned to the hospitals, some gets passed to the data
Quality Team as a whole, but it is not a specific person’s role to do
this. It is handled within the team according to workload and urgency and
so there is no way of giving a ‘whole time equivalent’ figure on this.
