b)Can you confirm if South East Coast SHA carried out their own Serious Untoward Incident (SUI) investigation into the Care and Treatement of Samuel Reid Wentworth?

c)If yes, when was this Report published and in the public's interest, can I have a copy please.

Maybe a few of us could develop a database of evidence,(not just MH complaints but all 'inquiries' and current SUI investigations) in a structured manner showing why NHS fails to learn lessons.Current Report: http://www.southeastcoast.nhs.uk/News%20...
