Dear Greater Manchester Mental Health NHS Foundation Trust,

I work for Lancashire County Council as a Social Care Support Officer, and have been allocated a lady with a mental health diagnosis.

As she now lives in the Lancashire area after moving from Salford/Manchester many years ago, many of her mental health notes have not been forwarded.

Under the Freedom of Information Act the Chorley and South Ribble Community Mental Health Team require details of her admission to Meadowbrook Unit. She was admitted over twenty years ago and stayed a patient for a prolonged amount of time i.e. over 6 months. Her husband is unsure if she was detained under a Section 3 at this time.

I would be grateful if contact could be made with me via secure network e.g. email or telephone so that her personal details can be relayed to you and information provided if possible.
