Thank you for your emails requesting confirmation of my initial response.
Our Head of Corporate Governance has confirmed that, when Derbyshire County PCT staff transferred to Derbyshire Community Health Services, all records were transferred with them as part of the process. Therefore NHS Derbyshire Cluster do not hold the information that you have requested.
