GEB 1 VOL 1 - Further Information

The request was refused by Department of Health and Social Care.

Dear Department of Health and Social Care,

I refer to information you provided in (Your Ref) FOI-1118059. This request is specifically in relation to the file volume GEB 1 VOL 1.

In your original response you state that "This file was requested from storage and booked out to a member of staff but has not been returned to storage."

In this new FOI request I would please like to know:

a) When was this file volume requested from storage?
b) When was this file volume booked out to a member of staff?
c) Has DHSC asked that member of staff to return the file, and if not, do you plan to do so?
d) If DHSC has asked that member of staff to return the file, when did this happen and what was the outcome of that request?

Yours faithfully,

JJ Evans
