Dear Freedom of Information Officer,

I am writing to make a request under the Freedom of Information Act and the Environmental Information Regulations for information on asbestos.

More specifically I am requesting the following:

1. a) How many new cases of asbestos related illnesses were recorded in 2016. Please provide a breakdown of the ages of the individuals at time illness was recorded, their gender; the type of asbestos-related illness (eg pleural plaque with the presence of asbestos; pneumoconiosis due to asbestos and other mineral fibres; mesothelioma) and the hospital/health care setting within which the illness was recorded.

I have provided an example of how this answer might be formatted:

AGE GENDER TYPE OF ILLNESS LOCATION

56 Female Pneumoconiosis Eastern hospital

This list is not exhaustive. (eg if another type of asbestos-related illness is identified or the recording took place in a non-healthcare setting please include that information in the table).

5. a) How many deaths due to asbestos related illnesses were recorded in 2016. Please provide a breakdown of the ages of the individuals at time of death, their gender; the type of asbestos-related illness (eg pleural plaque with the presence of asbestos; pneumoconiosis due to asbestos and other mineral fibres; mesothelioma) and the hospital/health care setting within which the illness was treated.

Please include the address of each building, when survey was conducted and if a record has been made of the type/degree of asbestos presence, whether the asbestos present poses a risk to health of staff and/or patients please include this information as well as any other relevant information.
