Footways width, suitability for wheelchair users

Under the freedom of information act I make the following request.

What guidance or standards do the council follow with respect to consideration of the design and suitability of existing footways i.e. width, kerb height, slope etc in a small village without speed protection (i.e. no speed cameras, road calming/narrowing etc) where it was recognised in previous traffic surveys that up to 80+ percent of all drivers exceeded 30mph

What is the process for getting a council examination of a footway that does not support the Disabilities act, footway is too narrow for a wheelchair user to use, no dropped kerbs to allow passage around a narrowing footway.

Who is responsible for the risk assessment regarding use of narrow pavements unsuitable for wheelchair or pedestrians with limited mobility at sites without proper speed reduction measures.
