Dear Public Health England,

Matt Hancock was unable on television to provide this data and said he was not sure it existed He then recommended your organsation to provide this data please.
Please provide asap all recorded information and documentation re fatalities from Covid 19 vaccines -Pfizer Moderna and Astrazeneca wirhin 28 days of vaccine being taken.Please advise age per fatality and name of vaccine per fatality.

Under Section 16 of the FOI Act, public authorities have a duty to provide advice and assistance. Accordingly, you may wish to contact the Office of National Statistics (ONS), which is responsible for the registration of death certificates in England and Wales. Deaths that have been directly attributed to the vaccine due to an adverse reaction can be found within the ONS Monthly Mortality Analysis. Additionally, you may wish to contact the Medicines and Healthcare products Regulatory Agency (MHRA). The MHRA regulate medicines and medical devices, and ensures that medicines and medical devices including vaccines, meet applicable standards of safety, quality and efficacy. The MHRA operate the Yellow Card scheme, which is the UK system for collecting and monitoring information on suspected safety concerns or incidents involving medicines and medical devices.
