DNACPR and Vaccination statistics for Covid-19 deaths
Dear Pennine Acute Hospitals NHS Trust,
As of 4pm on 11 October 2020, NHS England reports that 595 patients have sadly died in your hospitals after testing positive for COVID-19.
1. How many of these 595 patients had DNACPR orders (or similar decisions) on their records?
2. How many of these 595 patients had been given a flu vaccine?

Please pass this on to the person who conducts Freedom of Information reviews.
I am writing to request an internal review of Pennine Acute Hospitals NHS Trust's handling of my FOI request 'DNACPR and Vaccination statistics for Covid-19 deaths'.
You have estimated that the time taken to review 595 patients' records for DNACPR orders 'would be over the timelines expected with the Freedom of Information Act'. This is both unreasonable and evasive given that details about individual patients are both routinely collected and published (ie. sex, age, ethnicity, underlying health conditions, etc.) If information about DNACPR orders are recorded on a database, generating a report filtered to these 595 patients would be a few minutes work. If that's the case, please review your decision to decline my FOI request on this matter.
If the information is only held on a hard copy paper record for each individual patient then please confirm that this is the case. Also please confirm that this information will not be entered into a database within the next 12 months.

Dear Chris,
Thank you for your email. Do not Resuscitate orders are recorded on a lilac paper self-carbonating form and not coded under ICD-10. The forms are then saved in individual patient records. Therefore, it is not possible to run a report to retrieve the data. Pennine Acute Hospitals NHS Trust does not have an Electronic Patient Record system in place but still uses a Patient Administration System. I can also confirm that the information will not be entered into a database.

Thank-you for the swift response. If you do not have time to check 595 records, perhaps you have time to check a reduced sample of 20% of those records and report back how many of them contain a piece of lilac paper? As this does not require a lengthy review of each record and merely a glance, this should be a very swift process. Please let me know either way.

Apologies for the delayed response. Our End of Life Care team will manually review the notes from Quarter 2 (July - September) of all patients who have died with a COVID-19 diagnosis to establish whether a DNACPR order was in place.
I will keep you updated on the progress of your request but please be aware that this may take longer than normal as our clinical teams are stretched at present

Thank you kindly for your response. I have had several responses from different trusts providing this detail, including London North West University, which involved 623 patient records. They were able to report that 555 patients had DNACPR orders (or similar decisions) on their records.
I look forward to receiving similar detail from your End of Life Care team for Quarter 2 and would appreciate an update on timescales.

Many thanks for your email and please accept our apologies for the delayed response. As advised, the clinical teams were having to review the notes manually. Other Trusts may have been able to provide the information fairly easily if they have an Electronic Patient Record system in place. Unfortunately, Pennine Acute Hospitals NHS Trust does not have an EPR at present so would have had to carry out a manual review on records.
The information is now ready for release and I have attached a response to the internal review you requested.
