Our Response :

 

There is a diabetes care pathway in Derbyshire.  However it is an NHS
pathway, care pathways are NHS terminology.  DCC via Public Health
contributes to the relevant elements of the pathway (prevention and early
intervention), but it is healthcare focused and sits with the Clinical
Commissioning Groups (CCG’s).

Thanks Neil,
This therefore confirms that knowledge is essential of the management of diabetes so therefore part of the equality act to provide appointments and training. People haven't understood the seriousness of this condition and the pandemic that is occurring as is a hidden disability but affects everyday life.
What is available in the north of the county?
The CCGs produce the care pathway as you state but social care provide the care - joint working? Therefore please explain how this works? Sorry I can't seem to completely comprehend how Social Care determine its strategy.

Derbyshire County Council via its Public Health functions works with NHS
Commissioners, Providers and other relevant agencies to deliver a
comprehensive prevention, early identification and treatment for our
population of diabetics and those at risk of diabetes (in particular type
2 diabetes).

 

Attached is the South Derbyshire diabetes pathway, the North Derbyshire
pathway is currently being refreshed and will be finalised shortly, if you
wish to see a copy of this, then a request should be made to NHS North
Derbyshire CCG.

 

Currently in Derbyshire the CCGs commission secondary care to provide
services for people with type 1 diabetes and for those with complications
with type 2 diabetes.  People with type 2 diabetes are expected to be
managed within primary care.  The CCGs commission structured education
programmes for people newly diagnosed with type 2 diabetes, whilst people
with type 1 diabetes attend programmes at the hospital eg DAFNE; Dose
Adjustment For Normal Eating – a way of managing type 1 diabetes in
adults.

 

Derbyshire County Council Public Health Department commission
self-management courses (previously known as the expert patient programme)
for people with long term conditions.  This is a 6 week programme covering
many elements of self-management including lifestyle, (diet, nutrition,
exercise, stopping smoking), prescribing, goal setting, expectations,
where to seek advice other than the GP, relaxation, coping strategies etc.

 

Public Health currently leads the work on the National Diabetes Prevention
Programme on behalf of the LA and CCGs within Derby City and Derbyshire
County.  Patients who are eligible for this programme will have a blood
sugar level within defined parameters and will be invited to attend a
programme involving nutrition advise and exercise over a 9 month period. 
The programme aims to reduce the numbers of people developing diabetes
through changes in lifestyle.

 

The Public Health Department also works to reduce risk factors associated
with diabetes, for example by promoting healthy eating and physical
activity linking to national campaigns where appropriate.

Thanks Neil, you say it's being refreshed but as far as I am aware there wasn't one and the pandemic is a modern society problem that the country as a whole is having to deal with therefore I conclude that until campaigns and public knowledge was updated it was more of the individual living with the long term condition self awareness not what are the local authority going to have to have responsiblity for.
But we all know the whole world is about money not people.
