Please answer the following questions;

1) Supply the name of the office/department that provides your office with statistics for causes of death from each NHS Trust Boards in England and Wales

2) Supply the statistics for the number of death figures for in-patients who tested positive and died primarily of COVID-19 within Shropshire NHS Trust hospitals for 2020

3) Supply the statistics for the number of death figures for in-patients who tested positive and died primarily of COVID-19 within Shropshire NHS Trust hospitals for 2021

4) Supply the statistics for the number of Medical Death Certificates issued for in-patients who tested positive and died primarily of COVID-19 within Shropshire NHS Trust hospitals in 2020.

Thank you for your response to my questions, however it is unclear andf confusing as to the difference between NHS Trust Hospital and hospitals.

1) Why can you not provided statistics for ALL hospitals per se in the area requested

2) Why the division between NHS Trust and "Hopsitals" - please define?

3) How can you provided accurate statistic when you are elliminatinng NHS TRUST Hopsital?

4) According to my research the following are the only hospitals in shropshire, appart from one or two that are private the rest are NHS Trusts
Shrewsbury and Telford Hospital NHS Trust
Shropshire Community Health NHS Trust

Therefore please can you answer the following

However the Trust is able to confirm that 0 patients died while an
inpatient.
In answer to my previous FOI questions here below:
1) Why can you not provided statistics for ALL hospitals per se in the area requested?

4) According to my research the following are the only hospitals in Shropshire, appart from one or two that are private the rest are NHS Trusts:

In answer to my question:
Who supplies ONS mortality data?

You answered:
As advised in the previous FOI, ONS data is supplied by the General Registry Office using information taken from the death registration process. It is supplied to us by the Registration Online System (RON). We do not receive mortality data from NHS Trusts boards in England or Wales.
You answered:
From 2 January 2020 to 1 January 2021, there were 208 COVID-19 deaths registered in hospitals in the Shropshire Local Authority Area.

Therefore taking into consideration the above responses you made, we note that there is a discrepancy between the death figures from COV19 for all hospitals in Shropshire that you provided, which you allege were 208 for the above period. hospitals?

 

Our response:

​Thank you for your enquiry.

Hospitals are defined as communal establishment within the place of
occurrence category, not as a geographical category. As a result of
improvements in the classification and coding of communal establishments,
the place of death definition that we use was revised in 2011. These
changes were implemented for 2010 mortality statistics. In particular, the
classification was changed to specifically identify local authority and
non-local-authority care homes. The categories for NHS and non-NHS
psychiatric hospitals, other hospitals and communal establishments for the
care of the sick have also been replaced with a category for all
hospitals. This reflects current user needs.

The groups used for the place where death occurred are:

o Home (those at the usual residence of the deceased (according to the
informant)‚ where this is not a communal establishment)
o Care home (whether local authority or non-local authority)
o Hospitals and communal establishments for the care of the sick
(excluding psychiatric hospitals and hospices) (whether NHS or other
than NHS)
o Hospices (whether NHS or other than NHS)
o Other communal establishments include schools, convents and
monasteries, nurses' homes, university and college halls of residence,
young offender institutions, secure training centres, detention
centres, prisons and remand homes elsewhere: includes all places not
covered above such as deaths on a motorway; at the beach; climbing a
mountain; walking down the street; at the cinema; at a football match;
while out shopping; or in someone else's home

To calculate death figures by place of death, we use information collected
on the death certificate to determine where the death occurred and where
the deceased lived. When these two variables are the same location, and
this location is not a communal establishment, we create the group for
deaths at home (sometimes referred to as "private homes"). Where the
individual died in a communal establishment, we link the mortality data to
a communal establishment file via a unique identifier assigned by the
registrar. This allows us to collect information on the type of
establishment, by breaking the list into over 80 types of organisation.
Using these, we group the organisations into the categories described
above. Where the deceased did not die in their own home, or in a communal
establishment, they are included in figures for "elsewhere", which covers
all other locations.

We do not hold the geography which would constitute an NHS trust, as we do
not know which hospitals are managed by each named NHS trust.  We also do
not distinguish on place of occurrence by NHS or private hospital in the
publications we have referenced in your previous FOI:

Deaths published in our [3]Deaths by place of occurrence and local
authority publication are published in line with the [4]National
Statistics Postcode Lookup which is based on the area of usual residence
of the deceased.

The "local authority" is the local area in which the deceased resided.

For example, if the deceased lived in Abergavenny, (local authority
Monmouthshire,  but was taken to a hospital based in Newport  (local
authority Gwent) the death would be recorded under Monmouthshire, but the
place of occurrence would be  listed as a hospital death. To clarify, the
death would be counted under the local authority where the deceased lived,
not by the postcode of where the deceased died.

This would explain the difference in our data as we are using different
methodology to NHS Digital.  

Access to data for media

ONS data is published weekly by cause and age band in Deaths Registered
Weekly in England and Wales and by local authority and place of occurrence
in [5]Deaths by place of occurrence and local authority .

All analyses are freely available on the [6]ONS website; for details on
how we disseminate the statistics see [7]Section 5, Accessibility and
clarity subsection.

However, the [8]UK Summary | Coronavirus (COVID-19) in the UK dashboard
which is also frequently referred to by the media is managed by UK Health
Security Agency (UKHSA).  They can be contacted

COVID-19 deaths by named hospital

As advised, we do not hold analysis of COVID-19 deaths by named hospital
or based on NHS trusts. The NHS do produce [10]COVID-19 deaths by NHS
trust. These statistics are published every weekday. If you would like
further information, they can be contacted via email

We can provide a custom output of deaths due to COVID-19 by other health
areas such as Clinical Commissioning group, Welsh Health Board or
Sustainability and Transformation Plan (STP) which are the newest NHS
Health geography available. This is a larger area of output than Clinical
Commissioning groups.

We can also distinguish between NHS and private medical establishments
within this output. This would be subject to legal frameworks, disclosure
control, resources and agreements of [12]costs, where appropriate. Such
enquiries would fall outside of the Freedom of Information regime and
