Dear Coventry Road Surgery

I would be grateful if you could supply the following information
as of today‘s date:

1/ the number of patients registered at your GP’s practice;

2/ the number of patients whose records have had the objection code
9Nu0 entered, following the patient declining their consent to
their clinical data being extracted electronically;

3/ the number of patients whose records have been coded with 9Nu4
forbidding any of the patient's data, from any NHS source, leaving
the Health and Social Care Information Centre (HSCIC);

4/ the number of patient subject access requests (Data Protection
Act 1998, section 7) received in 2014;

5/ fees quoted for subject access requests in 2014;

6/ number of patients, to date, who have registered to obtain their
medication records online.

It appears you may be unaccustomed to Freedom of Information requests. I will need to refer you to the Act itself and the Code of Practice, but first I am forwarding your email for administrator attention. I will get back to you.

Good morning Carol

I am very sorry to trouble you but I am advised to use Coventry Road Surgery's internal review process because the practice asked for a fee to provide information I requested under the Freedom of Information Act 2000 about data protection of health records.

78 practices have responded in full and a further 24 practices are in the process of responding. Only Coventry Road Surgery has asked for a fee.

I understand the practice may be unaccustomed to receiving Freedom of Information requests, but that does not affect the practice's legal duties. There should be a procedure for internal review in place carried out by someone independent from the original decision. If you do not have anything like this set up, please let me know and I will keep you informed.

Alternatively, please provide the information I requested without charge.
