I would be grateful if the Trust can consider the following requests for information under the FOI:
1) How many credit cards or debit cards are issued to the Trust?
If there are such cards, are they registered to an individual eg the Director of Finance or others?
Is there guidance or a procedure showing the circumstances under which any cards can be used?
From what statement dates are the individual transactions for each card held?

Dear NUH,
I requested information through the whatdotheyknow website on the 4th May and followed up with a reminder on the 16th August - see copies below.
I have not received acknowledgements or a reply.
I have checked the whatdotheyknow website and this uses the FOI@nuh.nhs.uk email which is also listed on the Trust website.
I also note that the NUH has replied to other requests on the WDTK website during the May-August 2012 period, including a 15th August 2012 reply.
I would be grateful if the Trust can email me a copy of the Trust FOI procedures and its complaints policy.
If I do not receive a reply within the next 4 weeks, I intend to contact the Information Commissioners Office.
