Dear Yorkshire Ambulance Service NHS Trust,

Hi,

On 13/12/2015 at 02:47am I made a 999 call as my father was having a heart attack, unfortunately my father did not make it and passed away. I was wondering if I could help me get a copy of the recording for me and my family.

If you need any more information please let me know.

Yours faithfully,

Jack Kanyama
