Dear FOI, England (NHS ENGLAND),

To clarify, my name is Dave Jack Hammer but I prefer to be called Jack.

I don't want to post my address on a public forum. In fact What Do They Know specifically says not to do this but as Im happy with email correspondence, this should be fine.

I look forward to the answer to my original query.

Thanks

Yours sincerely,

Jack Hammer
