Dear Western Cheshire Primary Care Trust,

Please can you provide me with the following pieces of information/documentation-

A copy of the trusts policies for making continuing decisions

Advise what training individuals making decisions about funding receive and who requires that these individuals require this training, is it a Dept of Health requirement or the trusts own policy?

Can you also provide a copy of the training materials used.

Thank you for your e-mail below, requesting information under the Freedom
of Information Act 2000.

In answer to your specific questions, please see the information below:

Please can you provide me with the following pieces of
information/documentation about Continuing Health Care-

1. A copy of the trust's policies for making continuing health care
decisions;

Please find attached a copy of the National Framework and the
Decision Support Tool

2. Advise what training that individuals who make decisions about
funding receive and who requires that these individuals require this
training; is it a Dept of Health requirement or the trust's own
policy? Training is provided for professionals who complete the
Continuing Health Care paper work. Recommendations on eligibility are made
by members of the multi-disciplinary panel, which are usually managers
from health and social care.

3. Can you also provide a copy of the training materials used. Please
see the attached PowerPoint presentation.

4. The number of applications made to the trust for continuing health
care for 2008, 2009 and 2010.

There are a higher number of applications in 2008, due to the
introduction of new criteria in October 2007.

health care for 2008, 2009 and 2010.

6. How many decisions were appealed relating to applications made to the
trust for continuing health care for 2008, 2009 and 2010 and the outcomes
of and reasons for those appeals?

For all cases from 2008 - 2010, the reasons for the appeals were
that the individual or representative did not agree with the panel's
recommendation about eligibility.

2008 We are currently unable to provide a comprehensive list
of stage 1 disputes for this period.

Stage 2 disputes 27 cases.

Outcome: 23 Continuing Health Care agreed; 1
referred to out of area Primary Care Trust and 3 resolved or no further
dispute.

2009 Total: 36 disputes.

Outcome: 2 Continuing Health Care agreed;
1 awaiting Independent Review Panel outcome; 1 referred to out of area
Primary Care Trust; 17 resolved or no further dispute and 15
cases awaiting panel
hearing or correspondence from individual/representative.

2010 Total: 58 disputes.

Outcome: 9 Continuing Health Care agreed; 2 referred
to out of area Primary Care Trust; 16 resolved or no further dispute; 24
cases awaiting records, panel hearing or correspondence from
individual/representative; 2
awaiting outcome of Local Dispute panel and 5 cases at stage 1 (awaiting
stage 1 review).
