Construction Deaths at the Olympic Park
Dear London Legacy Development Corporation,
In 2012 the Olympic Delivery Authority, which I gather you have replaced, was given an award for which it has since been widely lauded - an award for having not a single construction-related fatality on the Olympic Park. However, I note 2011 reports such as the below suggesting a crane operator died in June 2011.

Can you please provide the health and safety reports from this incident, and confirm why it was not considered a construction-related death? Alternatively, please explain how the Londonist report is inaccurate.
