Under the Freedom of Information Act I require a formal response from the Police & Crime Commissioner’s Office to the following questions:-

1. How many complaint has the Gwent Police and Crime Commissioner’s Office received relating to the Chief Constable of Gwent Police, Jeff Farrar in the years:-

a) 2014
b) 2015

2. How long on average did it take the Gwent Police and Crime Commissioner’s Office to conduct the investigation(s)?

I am making the above request following the Gwent Police and Crime Commissioner – Ian Johnston refusing to acknowledge correspondence sent by me, Mr M Williams dated 4 March 2015, relating to a complaint made against Chief Constable Jeff Farrar in July 2014:-

Dear Police and Crime Commissioner for Gwent – Mr Ian Johnston

Further to my correspondence dated 24 February 2015.

A representative has written to the Aneurin Bevan Heath Board (ABHB) regarding the illegal accesses into my personal / medical records held by the ABHB / NHS.

A personal response has been received from Judith Paget – Chief Executive ABHB.

I have attached a copy of Judith Paget’s letter with the relative information showing. I have removed other information, at this moment in time, as not to cause a Data Protection as the information involves another patient.

You , Ian Johnston will clearly see, Judith Paget refers to, “I can also confirm that the Criminal Justice Liaison Service (CJLS) received a referral to perform an assessment (for Mr M Williams)”

As the law states and you Mr Johnston are aware:-

A CJLS identifies people taken to a police station or court who suffer with mental health issues, a learning disability or any other identified specialised needs. Once identified, they can be directed towards an appropriate service.

I HAVE NEVER BEEN ARESTED, NEVER broken the law, NEVER participated in any illegal acts, I am NOT a threat to anyone / the community, the CJLS had no legal right to access, violate my personal records.

Considering Chief Constable, Jeff Farrar refused to investigate my complaint of illegal access involving the CJLS and now Judith Paget has confirmed that the relevant authority contacted the ABHB for an assessment of myself, Mr M Williams. This has highlighted the police have created a false criminal record against me, Mr M Williams.

As the Police and Crime Commissioner, elected by the people for the people, I require you to:-

· Investigate who within the police force created a false arrest, a false questioning, whatever false excuse was recorded within police records against me, Mr M Williams that initiated the ABHB to perform the alleged assessment of myself, Mr M Williams.

Please remember the ABHB has recorded these illegal actions within the ABHB’s official records. I must state, I have NEVER been assessed by the CJLS.

· Investigate who was involved within the police force, etc., why they committed the offences against me, Mr M. Williams, etc. As the victim, I have every right to know these facts.

· Provide written proof of the crime number that relates to the involvement of the ABHB / CJLS’ assessment of me, Mr M Williams and the alleged crime I was allegedly detained for, alleged arrested for, etc.

As well as continue to investigate the complaint I submitted to you Mr Johnston concerning CC Jeff Farrar and his refusal to investigate my complaint submitted to CC Farrar dated 10 July 2014, regarding the illegal accesses, distributing of my personal / medical records.

I await your acknowledgement of this correspondence, which has attachments of further evidence and confirmation that you, Mr Johnston will be attending to this complaint of corruption as matter of urgency. Please remember you the Police and Crime Commissioner, Ian Johnston were made aware of falsification of official records against me Mr M Williams, in July 2014.

Two duties are placed on public authorities under Section 1 of the Freedom

Under the provisions of those sections of the FOIA, we can confirm that
