I am writing to request information under the Freedom of Information ( Scotland) Act 2002. In order to assist you, I am outlining my query as specifically as possible.
I would like to know:
The number of complaints made to your organisation either by patients; or on their behalf about the maltreatment of patients detained under the Mental Health (Scotland) Act since January 2014. Specifically regarding:
- Perceived physical abuse by staff, whether carers, nurses or doctors;
- Perceived psychological or emotional abuse by any of the above;
- The number of investigations opened during the above time period by the health board or any external agency of which you are aware as a result of receiving a complaint.
I would prefer to receive the information in the format you hold it. An exported file such as .xls or .csv from a database would be ideal. If providing this information is not possible then please explain why you cannot supply it.
If my request is denied in whole or part, I ask that you justify all deletions by reference to specific exemptions of the act.
