1. Has this Trust ever deployed the Civica Paris product?

If the answer is "no", please go to question 4

2. If you answered "yes", then please choose the applicable option(s):

a) Yes, we have deployed Civica Paris & it is still in use. We have no plans to replace it

b) Yes, we have deployed Civica Paris & it is still in use. We may replace it in the next five years

c) Yes, we have deployed Civica Paris in the past. However, we are now in the process of decommissioning it

d) Yes, we have deployed Civica Paris in the past. However, we are planning to decommission it in the next five years

e) Yes, we have deployed Civica Paris in the past. However, we have since decommissioned it (please specify date)

Further to your recent request for information under the Freedom of Information Act 2000 (FOI). Over the past few weeks Leicestershire Partnership NHS Trust has received a number of requests in a similar format.

Firstly, we have concerns regarding the validity of the requests. Leicestershire Partnership NHS Trust should like to remind you that under S.8 (1)(b) of the Freedom of Information Act, to make a valid request, you are obliged to provide your real name. Given the similarities in the requests we suspect aliases have been used. The Trust is happy to process your requests, however, given our concerns, we are writing to advise that your requests will not be processed without confirmation that you have indeed provided your real name. In responding, we ask you take notice of point (19) of the Information Commissioner’s Office (ICO) guidance - Recognising a request made under the Freedom of Information Act (Section 8) - https://ico.org.uk/media/for-organisatio.... The guidance states that the ICO will not consider any complaints where a requester has used a false name.

Secondly, in the event that you should provide proof of your identity and the requests are deemed valid, we will aggregate the requests we have received from you in the last 60 days and consider the appropriate costs limit under Section 12 of the Freedom of Information Act 2000.
