CHC Funding

May I ask that you clarify the following:

* What time period are you seeking information for? i.e. the past 6,
12, 18 months etc? I would like information pertaining to 18 months ago

* "…in this area." Does this mean the geographical area covered by
Milton Keynes CCG? Please note this is unlikely to be the whole of
Milton Keynes. Yes the area covered by MK NHS commissioners
* "… offered CHC funding with a dementia diagnosis” – does this mean
you are seeking the number or individuals diagnosed with dementia and
who are eligible for CHC? Yes please

With regard to your request below, unfortunately Milton Keynes CCG require
further clarification.  Also it would not be possible to collate the info
in the format sought within the 18 hours obligated by the Act.  I have
explained in detail below.
 
CCGs do not intrinsically record diagnosis, i.e. dementia, in relation to
determining whether a patient is eligible/not eligible for Continuing
Healthcare. It may be recorded on the Decision Support Tool that is
completed at the time of assessment and sometimes it is not recorded at
all. 
 
In order for MKCCG to check if the diagnosis information is held it would
be necessary to manually assess the individual records; this would take in
excess of 18 hours.
 
Continuing Healthcare is a process of identifying whether a patient has a
Primary Healthcare Need (PHN) by following the National Framework for NHS
Continuing Healthcare and NHS-Funded Nursing Care guidelines. There could
be mention of the term “dementia” within the notes of the Decision Support
Tool document for each individual patient which is completed by the Nurse
Assessor as part of the decision making process, but would, if at all,
only be mentioned when giving a brief medical history of the patient.
 
To assist you further I can advise, as detailed above, that information is
not recorded by a patient’s diagnosis, however it is recorded within the
following:
 
•       Learning Disabilities
•       Mental Health (please note that within this category patients
would have a dementia diagnosis but some Physical Disabilities would also
have Dementia alongside say Parkinson’s etc. which would further compound
searches)
•       Physical Disability
 
Also your request for information relating to “18 months ago” is
ambiguous,  we are unsure what period you want us to cover – it says 18
months ago which would be 2nd May 16 but we wonder if you require a
specific period?, e.g. the first quarter of 2016/2017 We could provide
number assessed (DST) v number found eligible?
 
Additionally to assist you to refine or reduce your request, should you
wish to resubmit one, I would advise that, if you were able to clarify
and/or refine your request to higher level data such as those listed
above, we may be able to supply information.  Once we are in receipt of
your refined request we will be in  a position to progress it.
