Dear Sefton Primary Care Trust (PCT),
Regarding continuing healthcare (CHC) applications made in 2011, but not including fast track CHC.
1. How many non-fast track CHC applications did you receive in 2011?
2. How many and what percentage of these applications resulted in a CHC fully funded package of care?
3. How many and what percentage of these applications resulted in a CHC particularly funded (e.g. joint with the local authority) package of care?
4. How many and what percentage of these applications resulted in a CHC fully funded placement?
5. How many and what percentage of these applications resulted in a CHC particularly funded (e.g. joint with the local authority) placement?
6. What is the average time taken from receiving a CHC application to an eligibility decision being made?
7. What percentage of those approved for CHC funding had a form of dementia?
8. What percentage of those approved for CHC funding was placed in EMI/Dementia settings?
9. How many people sit on your CHC panel? How many are health staff (e.g. nurses) and how many are social care staff (e.g. social workers)?
10. As part of the completion of the decision support tool does the nurse assessor visit the service user in person? If normal practice is to not visit the client in person please state why.
11. What was the total spending on CHC funded support for new claimants (i.e. total cost of support resulting from new applications)?
12. What was the total spending of CHC generally (i.e. all CHC recipients - existing and new)?
13. Is it normal practice for telephone interviewing or information gathering to be used as part of the decision support tool completion process?
14. How many and what percentage of denied/rejected/non-eligible CHC applications were appealed?
15. How many and what percentage of denied/rejected/non-eligible CHC applications which were appealed were subsequently approved?
16. What percentage of total CHC applications were fast track claims?
