Dear Ayrshire and Arran NHS Board,

I would be grateful if you can respond to this Freedom of Information (Scotland) Act request for your Health Board regarding insertion of cardiac pacemaker devices.

For your: cardiology, clinical governance, finance departments
Request: any already calculated data in the form of summary reports, audit reports, research reports or held information which may help respond to the questions below.

QUESTIONS BELOW

Question 1: Can you provide the insertion numbers for basic bradycardia pacemakers (i.e. single and dual chamber pacemakers) at your centre?

Time Period: Can you provide this data for the years 2011 to 2017. Can you provide this for the individual years. (where you have recorded this data)
• If this data is not recorded over a calendar year (eg. Jan-Dec 2011) but instead over a fiscal or training year can you still provide this.

Question 2: Can you provide the insertion numbers for implantable cardiac defibrillator (ICD) devices at your centre?

Time Period: Can you provide this data for the years 2011 to 2017. Can you provide this for the individual years. (if you have record of this data)
• If this data is not recorded over a calendar year (eg. Jan-Dec 2011) but instead over a fiscal or training year can you still provide this.

Question 3: Can you provide the insertion numbers for cardiac resynchronisation therapy devices (CRT) at your centre?

Time Period: Can you provide this data for the years 2011 to 2017. Can you provide this for the individual years. (if you any record of this data)?
• If this data is not recorded over a calendar year (eg. Jan-Dec 2011) but instead over a fiscal or training year can you still provide this.

Question 4: Can you provide the overall complication rate for implantation of these devices?
• Complication rate for simple bradycardia pacemaker devices
• Complication rate for ICD device implantations
• Complication rate for CRT device implantations

Time Period: Can you provide this data for the years 2011 to 2017. Can you provide this for the individual years (if you have any record of this data)?
• If this data is not recorded over a calendar year (eg. Jan-Dec 2011) but instead over a fiscal or training year can you still provide this.

Question 6: Do you regularly undertake clinical governance meetings regarding cardiac pacemaker devices? If the answer is yes:
• How frequently?
• Since which year?

The statutory deadline for us to respond to your request was yesterday. Please accept my sincere apologies for not meeting this deadline. We take any request for information seriously and we regret on this occasion we have fallen short of our normal standards in responding to your request.

Unfortunately due to seasonal pressures I am still awaiting information from the relevant department. Once I have received the necessary information I will be able to reply to your request. I would hope to have your response to you by the close of play tomorrow but certainly by the end of next week, at the very latest, but this does not remove your right to request a review.

As we have failed to comply with Part 1 of the Freedom of Information (Scotland) Act 2002 (FOISA), by failing to respond to your request for information within the timescale required by section 10(1) of FOISA, you have a valid reason to request a review. I have attached the Scottish Information Commissioner's (SIC) booklet - 'Your right to know'. To make an appeal to the SIC you must first ask us for a review. Page 23 gives examples of when you can ask for a review. I have also attached our complaints procedure leaflet.

Once again, please accept my apologies for any inconvenience caused by the
delay in responding to your enquiry. We take any request for information
seriously and we regret on this occasion we have fallen short of our

Failure Service, Golden Jubilee National Hospital. As we do not hold this
information it is classed as an exclusion under section 17 (1) of the
Freedom of Information (Scotland) Act 2002.

A4. We file the complications which relate to Infection/ lead/ header
problems, it does not include pneumothorax.  Please see answer to question
3 for how to obtain information for CRT device implantations.

* We have withheld any figure fewer than three (<3). We are unable to
break these figures down any further as we are bound by the laws of
confidentiality in respect of patients. It would be a breach of Data
Protection Act 1998 (DPA) to provide information that you could then
deductively use to identify patients. We believe to provide the exact
number may lead to these patients being identified which would breach the
first principle of DPA which is to process data fairly and lawfully. This
is classed as an absolute exemption under section 38 (1) (b) read in
conjunction with section 38 (2) (a) (i) of the Freedom of Information
(Scotland) Act 2002.

 

In coming to this decision we have taken into account the guidance
provided by the Scottish Information Commissioner (SIC) on applying
section 38 (1) (b) of FOISA,
