Can you clarify the process of making a claim for PIP /ESA

Dear Department for Work and Pensions,

A claimant makes a claim for PIP/ESA after completing the work capability forms or how your condition affects you form, then they are called for face to face assessment, the assessors taps away on his laptop as he asks specific questions, his report goes to DWP, and it is here that things get a little confusing, does the report the assessor sends to the DWP already have the points awarded, or does the DWP decision maker award the points?
