I would like to make a request under the Freedom of Information Act.

Could I please request for the last five years the calibration/service maintenance records for the ECT machine(s) at:
- Fairfield General Hospital ECT Clinic
- John Elliot Unit, Birch Hill Hospital
- Parklands House ECT Clinic, Royal Oldham Hospital
- ECT Department, Stepping Hill Hospital
- Buckton Building, Tameside General Hospital

Additionally, the make and model of the ECT device used, date of manufacture.

Further to your email below, I can now respond to your request dated 27^th
January 2020.

 

Following receipt of your request, searches were conducted within Pennine
Care NHS Foundation Trust (the Trust) to locate the requested information
and I can confirm that the information requested is held by the Trust.

 

Request

 

Could I please request for the last five years the calibration/service
maintenance records for the ECT machine(s) at:

 

- Fairfield General Hospital ECT Clinic

- John Elliot Unit, Birch Hill Hospital

- Parklands House ECT Clinic, Royal Oldham Hospital

- ECT Department, Stepping Hill Hospital

- Buckton Building, Tameside General Hospital

 

Additionally, the make and model of the ECT device used, date of
manufacture.

 

Please find attached the service maintenance records and also a separate
spreadheet, detailing the make, model and manufacture date of the ECT
devices.

 

You may notice that there are sections that have been redacted on the
service records. This is in line with section 40, Personal Data. The
reason for redacting the information is because it relates to staff
members and forms part of their personal information.

 

 

Section 40(2) - Personal Information represents information that
identifies a living individual, where disclosure of the information would
breach the principles of Article 5 of the General Data Protection
Regulation (2016) (GDPR).  Personal data is defined by Article 4(1) of the
GDPR and means any information relating to an identified or identifiable
natural person, who can be directly or indirectly identified from that
data. 

 

In accordance with the S.17 of the Freedom of Information Act 2000, this
correspondence is a Refusal Notice for that information.
