Breach of Health & Safety resulting in harm to a patient falling from the roof at Sandalwood Court, Swindon

1. A copy of the internal investigation report into the serious breach of health and safety at Sandalwood Court hospital in 2016 resulting in a patient that jumped from the roof resulting in serious injuries.
2.A copy of the HSE investigative report and recommendations associated with this event.
3.A copy of the report associated with the hearing held at Taunton magistrates court where a fine was imposed of £80,000 for serious breaches of health and safety.
