Dear NHS Bolton Clinical Commissioning Group,

I would like to make a request under the FOI Act.

For the purposes of the Act, please take the date of your receipt of this request as Wednesday 31st January 2018.

I read with interest about the Bolton Care Record (BCR) (http://www.boltoncarerecord.org/ ).

Please could you tell me:

•Is data derived from GP records extracted and uploaded to a 3rd party database (e.g. via Graphnet) or is it streamed in real time via Healthcare Gateway's MIG?

If uploaded, who is the data controller for the uploaded information?
Or are individual contributing organisations "data controllers in common" or "joint controllers"?

If uploaded, please could you provide me with a list of the read codes that, when present in the GP record, prohibit extraction and uploading to the BCR - for example, 93C1 (EMIS Web data sharing), 9Ndo (SCR optout), 9Nd1 etc

•Finally, are there any secondary uses (research, commissioning, "population health analytics") of personal data uploaded and held in the BCR?

I would be grateful if you would kindly acknowledge receipt of this request as recommended by the ICO (“It would be good practice to acknowledge receipt of requests and to refer to the 20 working day time limit, so that applicants know their request is being dealt with”).
