Dear Belfast Health and Social Care Trust (Northern Ireland),

Further to FOI/25709 where you have listed a table of locations for the blood patch procedures;

(1) As these locations were not outlined on patient records, which software was used to identify locations? Please provide a screen grab of the software/multiple software used so that we can see how data is entered. Please note, we are not asking for screen grabs for each of those patients as they can request this under SAR.
(2) In December 2015, patients were moved to the PTU from the ward. Who made that decision and why?
(3) Please outline all of the reasons 33 patients had their procedure in the day surgery unit? We understand one patient had 2 of theirs there to be filmed and a few had a CT guided blood patch. Please provide a breakdown.
(4) Please provide a breakdown of the different types of blood patches for each of the 159 patients i.e. CT Guided, Anesthetic used etc
(5) 5 patients had their patches performed in 2F. Was this following admission to A&E?
