Dear NHS England,

how many apms contracts for primary medical services are there in Leeds, Bradford and Dewsbury?

what is the contract price for each apms contract?

what is the duration of each apms contract?

can you provide copies of the winning bid for each apms contract currently being delivered?

Do the costs of premises from which the apms contracts are delivered form part of the contract price or are the premises provided by the nhs?

does the nhs place a formal tender for premises from which apms contracts are to be delivered?
