Dear East Midlands Ambulance Service NHS Trust,

I would be grateful if you could provide the following information:

1). Number of patients who were seen by South Central Ambulance Service between 01/06/2022 and 31/12/2022.

2). Number of unit hours actually deployed by the Trust between 01/06/2022 and 31/12/2022.

3). Number of adverse incidents reported Trust wide between 01/06/2022 and 31/12/2022. This question is referring to the date that they were reported not the date of the incident, however if the latter is easiest for you to obtain, that would be equally helpful.

4). Number of adverse incidents that have currently been open for greater than eight weeks.

5). Number of Patient Experience incidents (or local equivalent) received between 01/06/2022 and 31/12/2022 that have been upheld or partially upheld (or local equivalent). This question relates to those requiring investigation for learning and it would be helpful to exclude compliments received via the Patient Experience Team.

6). Number of Patient Experience incidents received between 01/06/2022 and 31/12/2022 that are open either pending investigation, or with investigations ongoing.

- Could you please also clarify exactly what you mean by 'adverse incident' it would be great if you could provide a definition of this.
- Could you please clarify what you mean by 'unit hours.'

If I do not receive the clarification within three months from the date of this email your request will be considered to have lapsed. (Under section 1(3) of the Freedom of Information Act, a public authority need not comply with a request unless any further information reasonably required to locate the information is supplied).

Thank you . Yes this request was meant for meant for East Midlands, apologies for the mistake.

By adverse incident, it means any incident that has been reported via the Trust's incident reporting policy or similar.

Unit hours refers to hours of resources that are deployed.
