Action plan for patients of unnecessary procedures

Dear Department of Health (Northern Ireland),

In July 2019, The 'Neurology Recall Support Group' reported to you urgent public safety concerns for patients attending the recall process. We addressed a number of concerns:

1. Recall consultant's letters had been changed after they had been written to reflect a lesser diagnosis, impacting on patients' follow-up care with their GP. We had requested that the audit logs be checked, in the interests of patient safety. Please can you advise if this was done?
2. We alerted you, that patients who underwent invasive procedures, were unable to access urgent care and were being sent home from hospital with sepsis/serious infections. Please outline what steps the department took to ensure the safety of those patients?
3. We advised you that the Belfast Trust failed to address blood patch patients during the recall process. We had requested to be seen by a blood patch expert. To date, this has not been done. What steps have you taken to ensure the Trust provides this?
4. On 01 June 2021, we again raised these safety concerns with the department and Trust board. No action was taken and in July another patient died after being sent home from the hospital twice, with a serious infection. The urgency in this matter is obvious in order to protect the lives of patients seeking medical attention. What action plan does the department have for the safety of those patients?

Thank you for your correspondence dated 13 September 2022, in relation to request for a Departmental internal review as regards to your unanswered request, “Action Plan for Patients of Unnecessary Procedures”, dated 16 August 2021, and copied below:

We apologise for the delay in response and acknowledge any frustration/distress this may have caused. It is with regret that my investigation into your request has shown that whilst an initial response was being drafted during August and September 2021, a response was never finalised and as such, the information is out of date.

We acknowledge that there has been a prolonged delay in advising your of this and once again extend our apologies. Whilst your request was made referencing the Freedom of Information Act (2000) as you were not requesting copies of records held by the department your request was not being managed as a Freedom of Information request; however, we further acknowledge that this was not communicated to you at the time. As a result, we have reviewed our policies in dealing with such requests and refined them, using this as an opportunity to improve our processes.

We would like to take this opportunity to invite you to consider if you would still like a response to the questions you posed in August 2021; we will endeavour to respond as quickly as possible should we receive a further request from you.
