I understand that you follow the "East Midlands Specialised Commissioning Group Policy on the provision of Gender Dysphoria services for adults" <URL:http://www.emscg.nhs.uk/Library/EMSCGGen...> which states-

"Appendix A

Commissioned Procedures

Gender reassignment involves a variety of therapeutic options, including surgical procedures and other clinical interventions summarised in Section 3.

The following procedures, if appropriate for the individual undergoing gender reassignment, should be routinely commissioned.

Donor site hair removal on surgeon’s recommendation"

Since this policy was ratified on 27th November 2009, please provide the total number of funding applications for such pre-operative donor-site/genital hair removal procedures (in accordance with a surgeon's recommendation and complied with the policy) that were-

a) successful, and

b) unsuccessful.

NOTE: As I am sure you will understand, in this case I have used a pseudonym to avoid the risk of "outing" my trans status.

Please note guidance from the Information Commissioner (January 2009) <URL:http://www.ico.gov.uk/upload/documents/l...> which states-

"Even when an obvious pseudonym has been used, as good practice a public authority should still consider the request even though technically it can be regarded as invalid. This approach could be adopted in cases where identity is not relevant to the request and, in view of the general principle within the FOIA of disclosure to the world at large, where the authority is content to disclose the information."

I hope you will act in good faith and consider my request. If this request is refused as invalid on the grounds of the use of a pseudonym, it will be resubmitted under the name of a valid organisation instead.
