For each of the academic periods 2018-19, 2019-20 and 2020-21:
a) How many serious incident investigations has Cardiff Metropolitan University conducted?
Al serious incidents are thoroughly investigated, with the final investigation report being scrutinised
by both the University's Executive Group and its Academic Board. Al investigations are fol owed by
action plans for improvement, the delivery of which are monitored through Academic Board.
Owing to a very low number of serious incident investigations, the University is invoking Section
40(2) of the Freedom of Information Act 2000 because to provide this information would risk
disclosure of personal data.
b) How many serious incident reports were produced?
c) How many serious incidents involved a fatality?
d) How many serious incident reports and/or their findings has Cardiff Metropolitan University
shared with other institutions?
e) How many serious incident reports and/or their findings have other institutions shared with
