•       Please can you share your serious incident reporting criteria and process 
•       Are all patients that are admitted to a caseload who die unexpectedly reported as a serious 
incident for the organisation?  
•       If not – what criteria do you use? 
 
Your request was received on and the Trust is managing it under the requirements of the Freedom of 
Information Act 2000. I am in the process of collating this information and hope to be in a position to 
respond to you shortly. 
 
In some circumstances a fee may be payable and if that is the case you will be advised and a fees 
notice issued. You will be required to submit your remittance before the Trust is able to proceed with 
your request.
