implementation of the revised National Framework for NHS Continuing Healthcare which

The NHS CHC & Complex Care service have plans in place ensure revised National Framework
is implemented effectively across all 5 CCG localities.
• Plans are being drawn up by Joint Operational Group to ensure all staff within the
service are aware of and understand these changes and are ready and able to change practice
on 1st October 2018.
• Those plans include actions required to inform partner agencies of the changes,
including Local Authorities, Community Staff, Integrated Discharge Teams, Hospice Staff, CWP
and Provider Agencies, ensuring they are able to make a referral when necessary in line with
the revised framework.
• Joint Operational Group are recommending to Programme Board that CHC referrals are
made using the IEG4 digital system from 1st October as this will ensure the updated CHC tools
are automatically used within the digital system thereby avoiding the potential for older
version of the tools being used.
• Steps have been taken by our digital partner to ensure the updated & revised tools are
available from 1st October enabling all staff to use revised tools with immediate effect.
• Support has been given to staff in partner agencies to register their staff on IEG4 in
readiness for 1st Oct 2018
• CCG Clinical Leads are working with leads in partner agencies to facilitate forums in
order to share knowledge so to ensure changes are widely communicated and implemented.
• Partners attending the Joint Operational Group are supporting with the facilitation of
such forums within Adult Social Care to ensure that updates are rolled out across both health
and social care sectors.
• Key changes to be communicated include:
1. Details of revised mandatory tools ( Checklist / Decision support tool / Fast track tool)
2. Key areas of policy that have been clarified
