Thank you for your information request indicated in bold below, which has now been
considered under the Freedom of Information Act 2000 by NHS Eastern Cheshire
Clinical Commissioning Group (CCG). I am able to provide you with the following

We would like to be sent a file containing the contact information the
Medicines Optimisation Team have for the GP Practices in your CCG.
Please supply email contact details, and / or fax numbers, for each
practice. Ideally the file you send should include data, for each practice,
for one or more of the following headings: GP Practice Code, GP Practice
Name, GP Practice Address, Telephone Number, E-mail Address and Fax
Number.

deal with your enquiry.
