clinical performance before and after the implementation of 60 hours consultant presence in the Labour Ward
Dear Ealing Hospital NHS Trust,
It is now a Clinical Negligence Scheme for Trusts (CNST) requirement for a 60-hours consultant presence at the Labour Ward with estimated deliveries of 2500-4000 deliveries per year.
Could you kindly provide the figures for the following measures, 1 year before the implementation of this act (-> 60 hours consultant cover in the labour ward) AND after its implementation (to this date), broken down to EACH month:
1. Number of births
2. Total number of instrumental vaginal deliveries
- number of failed instrumental deliveries
- number of successful instrumental deliveries
3. Total number of C-section
- number of elective C-section
- number of emergency C-section
4. Weekly hours of consultant cover on Labour Ward
5. Midwife/birth ratio
6. Supervisor to midwife ratio
7. Education and training programme attendance
8. Total cases of eclampsia
9. Total cases of maternal collapse
10. Number of ICU admissions in obstetrics
11. Total number of cases needing blood transfusions (divided into <2 units, >2 units, >4 units)
12. Number of postpartum hysterectomies
13. Number of cases of meconium aspiration
14. Number of cases of hypoxic encephalopathy (grades 2&3)
15. Number of SCBU admission
16. Number of NICU admission
17 Number of SUIs
18. Total no. of cases of massive PPH >1L
- no of cases of massive PPH >1L from vaginal del.
- no of cases of massive PPH >1L from C-sec
19. Total no of cases of massive PPH >2L
- no of cases of massive PPH >2L from vaginal del.
- no of cases of massive PPH >2L from C-sec

Some of these measures are in conjunction with those outlined in the Maternity Dashboard Card, while the rest are more detailed aspects but still relating to the measure included in the Maternity Dashboard Card.

There are few things that I am not entirely clear about, which would be great if you can help clarify further.
1. Under "type of birth" for every report, there are figures given for "% Neonates" - can you kindly further explain what exactly do these figures refer to? Is it the % of birth attended by the neonatal team for potential neonatal distress,etc?
2. Regarding number of complaints/SUIs, are these covering maternity i.e. obstetric ONLY or involve BOTH obstetrics and gynae?
In addition to these,
3. For the Apr 2011 - March 2012 maternity performance card, the absolute number of deliveries for each month were not provided (although a bar chart was given). Will it possible to get these numbers i.e. total number of deliveries by month between apr 2011 and mar 2012? This will be really helpful in helping me read the data more clearly, as the performance cards for the rest of the years provide these figures with gap missing in the middle for 2011-2012.

I have requested this information 40days ago and still waiting for a reply. It would be wonderful if you can provide me with further clarification of my queries at the earliest time that is convenient for you.
