RE: Your Information Request
I am writing in response to your request for information following a news article published on
the News Shopper website in relation the incident that took place at River House, Bethlem
Royal Hospital on 1 October 2012. We received your original request on 30 October 2012
under the Freedom of Information Act (2000).
We responded to your original request on 8 November 2012 and informed you that an
independent investigation was commissioned to help ensure a thorough understanding of
what happened and how such incidents could be prevented in the future. You followed up the
publication of the independent investigation report on 21 February and 11 June 2013 when
we informed you that the report was intended for future publication (Section 22 of the
Freedom of Information Act (2000)).
The independent investigation of the incident that took place at River House, Bethlem Royal
Hospital has been completed. The Trust is aiming to publish the report on 29 August 2013 in
the ‘Policy and Publications’ section of the Trust website under a sub-menu called ‘External
Reviews’. You will be able to find the ‘Policy and Publications’ section under ‘About Us’ when
you visit the Trust website at www.slam.nhs.uk
The information supplied to you continues to be protected by the Copyright, Designs and
Patents Act 1988. You are free to use it for your own purposes, including any non-commercial
research you are doing and for the purposes of news reporting. Any other reuse, for example
commercial publication, would require the permission of the Trust. This may be done by
writing to the Information Governance Office at the above address.
